How Extend Health Works
- Log on or call. Log on to
Sam's Club® Member Services or call toll free at 1.866.814.3637.
- Set your budget. Decide how much you want to contribute each year
to your employees’s health coverage. If you’re not sure, an Extend Health
representative can help. Once you’ve set the amount, the representative creates
the plan documents that let you give your employees the money — tax free.
- Your employees enroll. Extend Health helps you tell your employees
about the program and how to enroll. You and your employees log on or call Extend
Health to select a health plan from top insurance companies. You and your employees
can then use tax-free money to help pay for insurance premiums or qualified medical
expenses.
- Your employees are covered. Once your employees have been approved,
their coverage cannot be canceled for health reasons. And their coverage goes with
them wherever they work.
What Extend Health Costs
Other than the monthly amounts you provide to your employees, you pay a one-time
application fee of $200. This fee is waived for Sam’s Club Plus® Members.
If you haven’t yet upgraded to Sam’s Club Plus, your cost savings will
